- Deceased was killed by a runaway loaded tub in the Hardmine endless-rope haulage dip (inclination 1 in 5). He was working about 150 yards below a junction from a level. Before the occurrence scotch posts had been set as a protection from runaways. These scotch posts had been removed to allow a trolley of rails to pass and were being replaced. The load ran amain on the empty side, through the points at the junction not being turned as it was being brought back from a set of men beating up above the junction. Previously, the empty tub had been pushed up the dip over the points, and it was taken for granted that this put the points right for the return journey.
