- One of the cages came to the pity head level with a tram in the top deck a tub in the middle and another tram in the bottom deck; he took the tub out of the middle deck and then signalled for the cage to be raised so that the low deck was level with the flat sheets he got into it, contrary to the special rules, and the cage was then lowered to the low landing or 24 feet below in order that the trams might be taken out of the cage there. He had not got sufficiently far into the cage and was caught between the cage and the baulk carrying the fallers and fatally injured. He should have gone down some steps to the lower level.
