When engaged holing, in a place 6 feet 3 inches high, some coal fell from an exposed slip at the face and fatally injured Butler. A sprag set by Butler 5 feet from the fast side, where he was working, was displaced by fall. The holing was done after the fireman's visit, which was made about 9 a.m.
