- Some long timber was about to be sent down the shaft slung beneath the cage, and as was the custom on such occasions to place rails across the pit and load the timber direct on to a tram. Deceased was assisting to put a rail 12 feet long across the shaft at the top decking level of the pit bottom. The rail fell and jerked him into the shaft, and he fell 13 feet to the bottom of the cage hole. His skull was fractured and he died two months later on July 11th.
