- The deceased was attaching full tubs to an endless rope on a road rising at the rate of 1 in 7, and, either on account of his not having screwed the clip tight enough, or because he had got the clip on to a splice in the rope, two tubs filled with dirt, after going 30 yards ran back. He appeared to have been attempting to get into a refuge hole when he was caught by the tubs which dragged him about nine yards until they ran into a "jack catch" and tipped. He was so seriously injured that he died before being taken out of the pit. The clip was in working order and was, after the accident occurred, used on the same road and rope with three full tubs, and they were safely taken to the top of the plane, although the gradient on the way increased to 1 in 5. There was only one splice in the rope, where the two ends were joined together, and at that place the diameter of the rope was increased from 7 8 in. to 1in. Between the jack catch the tubs were caught by and the next one up the haulage road there was a distance of 90 yards. On a gradient of 1 in 7 they should have been one at least every 20 yards.
