- Deceased was in charge of a train of 16 empty tubs which was being lowered down a gradient of 1 in 12. He walked down in front of the moving empty train and on reaching the junction signalled for it to stop. He probably thought the train was going too fast. Immediately afterwards his leg was crushed against a brick pillar at the junction by the third tub of a train of twelve getting off the rails. The twelve tubs bad broken loose due to a fractured link at a point 60 yards away.
