- A truck loaded with calcined ironstone during the day and deceased gave instructions to the loader to lower the truck away to make room for a truck of slack, which was standing some little distance away. The loader eased the brake and the truck moved slowly down the gradient of about 1 in 80. He was on the opposite side to deceased, and on the truck stopping suddenly, he put the brake on and found deceased lying on the rails under the truck. He had evidently been trying to put a 5-ft. pit prop into the wheel as a sprag and it had got fast behind the axle guard and pushed him under the wheels. There was no need for a sprag as the brake was sufficient and his action was quite inexplicable.
