- He was set to drive return airway through the goaf, and had pulled out the pack wall and was then engaged in pulling out the dirt beyond, when a stone fell from a joint on to his back. Part of the joint was visible prior to the stone falling and he had had doubts as to the safety in working under it, for he had discussed its condition with another man. His judgement was at fault. He should have taken the stone down.
