He was killed by a fall of roof (sulphur and dogger), 5 inches thick, which fell away from slips. This accident took place in a whole working near the outcrop, where the roof stone would be likely to be treacherous. The seam was only 6 feet thick, and props could be set or re-set with ease. The roof fell over a space of 17 feet by 14 feet, and it was bad judgement to expose such a large area without sufficient support. He died two days later.
