When charging a vertical hole in the roof of a road with three cartridges of kynite by means of a wooden rammer when the charge exploded killing him and causing injuries to another man. The detonator probably got displaced and in pressing the charge to the top of the hole it came in contact with the side of the hole and was thus exploded.
