- A derrick pole was being erected to lift some material on to a building and deceased was employed in holding one of the guide ropes clear of a travelling chain. By pulling slack the steel rope coiled up and taking the cover off a switch-board, came in contact with a live terminal, Deceased, who was standing on wet ground, was electrocuted. The voltage was 440 and the men employed were aware of the danger of allowing the rope to touch any electrical wires.
