This accident was caused by a breach of Special Rule 103. Deceased, after having placed on the rails an empty tub that had become derailed while jigging, signalled to the jigger by pulling the bell line in the jig instead of getting into a manhole and allowing the taker-off to signal from the bottom in the ordinary way. He was caught by the moving tubs and crushed against the side of the jig.
