- Crushed between an empty tub and a collar under the roof in this top East Siding down the South incline. This road was nearly finished and was in none too good repair. The siding is rather cramped and they had got too many tubs there for convenience. This full journey was longer than usual and empties blocked the journey going out. Deceased and others were trying to move some of the empties off the rails to clear the journey he was between tubs and remained there after being told that the journey was going to be slackened back but unfortunately owing to a misunderstanding in signalling the journey was drawn up and he was leaning over at the end of the tub and was crushed against the collar.
