- Deceased, presumably, thought he had had the cage lowered on to the keps at the No.4 landing, and pushed in a full tub. The cage was, however, not on the keps, but was being held by the winding engineman by the foot-brake, and it at once descended. Deceased pushing the tub, was caught between the top hoop of the cage and the flat sheets, and crushed to death. The winding engineman said he did not think deceased was going to put a tub into the cage, and was therefore holding it with the foot-brake only. He expected deceased was going to ride up the shaft.
