- Deceased was standing on a scaffold using a long screw-key to loosen the nuts on the cover of a clack-box in a pumping pit. The key slipped off a nut and he overbalanced and fell over the edge of the scaffold which was unfenced, and covered only half the area of the shaft (9 feet in diameter). He was drowned in about 10 feet of water which stood at a level 10 feet below the scaffold.
