- Whilst engaged in building a chock on a longwall face in the Main Coal seam, a sudden weighting of the roof took place causing a fall of roof coal and dirt, which caught the deceased. The fall measured 6 feet 5 inches long and 3 feet 11 inches wide and 2 feet 7 inches thick, and came from a slip in the roof running parallel with the coal face. A bar Set 2 feet 11 inches from the face was knocked out by the fall.
