He was employed lashing empty tubs to an endless rope. He had to unlash and lash again on passing two mushroom pulleys which conducted the rope up to the tension wheel. He failed to unlash the chain, so unhitched it from the tub and on seeing the chain being taken up to the tension wheel he jumped through the fence and was dragged into the wheel. He ought to have stopped the rope before going through the fence. The bell wires were just above his head.
