- The cause of this accident was the wearing away of the insulation of an electric lighting cable at the washery by a wire by which the valve of a mixer was opened and closed, being improperly placed on the top of it by the deceased. It had originally been placed below the cables but it had been broken and replaced by the deceased. The wire crossed the cables at an angle of about 70 degrees and was pressing on the top cable and the result was that the friction caused the insulation to be worn away until the wire was in contact with the copper of the cable. He had been standing on a trough conveying coal and water from the mixer to the washer, and in which there was water, and touched the wire when it was alive, receiving a shock which killed him.
