- Deceased was attempting to pull the rope off the drum of a small haulage engine, which was carried on a staging above the tub road. To do this he stood on the top of a full tub which formed one of a set of tubs attached to another haulage rope. While standing on this tub the set was hauled forward and he was caught and crushed between the top of the tub and the underside of the staging. He received injuries from which he died on November 15th. There was no necessity for him to get on the top of the tub, and he had no right to do so.
