- He was walking along a gate when a fall of roof occurred and injured him so seriously that he died on 4th February, 1914. The stone came from a slip in the roof and one prop was displaced. Props were set at distances less than that stipulated by the timbering rules, but in the gate bars were used and, at the scene of accident, were some 20 feet apart. The timbering rule stipulates 9 feet. Had a bar been set the accident might have been prevented. The place had been inspected by a deputy three hours before and reported safe.
