Whitehead, under the superintendance of a deputy, was filling dirt in a journey where there had been a fall of side. One prop had been set to secure the road side and while room was being made to get up an additional support, another fall of side occurred which knocked Whitehead against the tub he was filling and inflicted injuries from which he died on the 11th November.
