- Week-end walling operations were in progress in the shaft, and deceased was descending on a small bowk which was guided by means of a hinged iron collar or shoe fixed on the upper on of the bowk and passing round one of the cage girder ropes, when, owing to deceased standing on the edge of the bowk, its base struck a gland on a column of pipes, causing the bowk to tilt and become wedged fast owing to the hinged collar or shoe gripping the guide rope. As the winding rope continued to pay out, the heavy dive hook attaching it to the bowk turned upside down and became detached, leaving the bowk fast to the guide rope. The normal clearance between the bowk and the gland was 16 inches. Deceased fell a distance of 27.5 yards on to the walling scaffold and received injuries from which he died on the 21st January 1911.
