- The stone after leaving the crusher, passed through a revolving screen into an iron shoot. Owing to the stone being wet and sticky it accumulated in the shoot. Deceased got into the shoot to release the material and in getting out slipped. He fell beneath the revolving screen which struck his head and fractured his skull. He ought to have used a long rake instead of getting into the shoot.
