- Deceased was employed as a hooker-on and signaller at a shunt in the Hard Mine Main engine dip. He also had occasionally to go down 60 yards to the New Moss shunt to send up a set of tubs. He was there at snapping time, and had signalled a journey away, but as it did not move he proceeded up the dip to ascertain the cause of the delay. When he had ravelled 15 yards, the tubs were put in motion and travelled up the dip at the rate of three miles per hour. Deceased arrived at the Hard Mine shunt, and a roadman saw him place a hand on the leading tub and then to slip against the tubs. He was dragged along for a distance of 12 yards and sustained serious injuries from which he died on the same day.
