The accident occurred in a gate mad in the Thick Coal. Deceased was in the act of setting a tree to support a bar which was reeling and while driving the head of the tree with a hammer the bed of White Coal forming the roof, together with the debris in the old works immediately above it, broke down and buried him. His body was recovered four hours later. The fall came from three previously unseen slips, smooth and converging in the White Coal and reeled out three settings of timber.
