- A bolt broke in the clutch between the drum and the pinion wheel and he went in to get it out. The end of the hauling rope at the time was 81 yards away and was attached to some tubs on an endless rope for the purpose of being taken further down the plane to be attached to a full train. The endless rope started and this caused the rope to be pulled and the drum and spur wheel to revolve and he was caught between the drum and bed plate and killed.
