Six lads out of ten had got out of the bottom deck of a double-decked cage, which was standing at the pit bottom flat sheets, when, as deceased was getting out, the cage moved down. He was crushed between the middle hoop of the cage and the flat sheets. The cages were standing held by the foot lever actuating a post brake on the winding-engine drum, the notched lever of which evidently slipped out from the holding down position, which, releasing the brake, allowed the cage to fall. No defect could be found in the brake lever, so it may not have been placed securely in position.
