- Deceased had been employed two mouths as a coal picker on a travelling belt in connection with the screens. His duty was to stand at the side of the belt and pick dirt out of the coal as it was carried past him. For some unknown reason, probably to amuse himself, he went to the end of the belt and by some means got his right foot trapped by the toes between two of the belt plates at the revolving drum end of the jib which is used to deliver the coal into the trucks. He clung to the balance weight chains with both hands while his left leg was torn off at the knee joint, and was found supporting himself on his right leg. First aid was rendered at the colliery, an operation followed, after which he developed symptoms of tetanus from which he died on the 18th October.
