He had gone up on to a small scaffold about 12 ft above this floor to oil the plummer of a horizontal shaft driving an elevator belt. The shaft was about 5.5 ft above the scaffold and made about 30 revolutions per minute. Deceased is supposed to have been reaching over the shaft to clean out the oil hole, when his sleeve was caught by a set stud, fastening a collar on the shaft close to the plummer. He was pulled under and over the shaft and killed almost at once.
