The accident occurred in the last lift a pillar in the second or broken workings in the Blake seam, which is made up of 3 feet 8 inches of top coal lying on a band of stone and bottom coal making a height of 7 feet in all. The band and bottom coal were worked forward about 4 feet 6 inches right across the lift which was loose on both sides. When the sprags were withdrawn the top coal fell nearly half way across the place from the right side. A shot was fired in the remainder but failed to bring all down a length of 6 feet being left up on the left side. While deceased’s marrow was preparing to drill a hole for a second shot deceased incautiously went under the part left hanging when it fell on him. The Local Inspectors reported ‘examined place where Ralph Waugh was killed and found the place safe.'
