- He was working in a wall 14 feet wide along with another miner, and when in the act of filling a waggon was killed by a fall of dogger and sulphur. Along the centre of the working there was a slight dislocation of 10 inches and the roof was entirely unsupported for a distance of 17 feet back from the face. A shot had been fired in the bottom stone only 20 minutes before and the roof had not been examined afterwards. The roof was damp and sufficient care was not exercised examining it, and timber should have been set nearer the face as the presence of the fault was well known.
