- The back shift had filled their tubs and run them on to the haulage dook as usual. The tubs were lifted by the fore shift in rakes of 6 or 8 and to each rake a bogie was attached. Deceased was on his way to his work and passed the rakes and when down some distance he heard tubs coming, and thinking it was empty tubs he stepped on to the load road and was knocked down by a runaway rake of loaded tubs. He died from his injuries four months after.
