- The deceased was turning round, after putting two empty hutches on to the cage, when he was struck by another empty, and knocked back into the shaft just as the cage descended, and was fatally crushed. A controller was provided to regulate the empties, and, by some means, this had evidently been half turned round, and so allowed another hutch to pass. Possibly Watson had only brought one hutch over the previous time and had forgotten to turn the controller.
