He was killed by eight full tubs which ran back on a gate dipping 1 in 9 towards the face. The front tub of an outgoing train got off the rails and, in order to get it on again, the train was lowered on to a sprag fixed behind the second tub. The coupling between the second and third tubs became detached and allowed eight tubs to run away. The accident was caused either by a corporal taking off the backstay to sprag the second tub or by the run rider failing to affix a backstay. The evidence was too conflicting to say which.
