Deceased was cleaning roads on Cousin’s engine plane dipping 4.5 inches in the yard. A tram ran wild from one of the headings and ran into a journey on its way to the shaft. Deceased was found underneath the second train. The runaway tram was being filled during the shift on a steep road, without having been secured otherwise than with sprags in the wheels, which were not sufficient to hold it.
