- At the beginning of the night shift some empty tubs were required inbye. Deceased clipped a journey of seven on to the endless rope at the bottom of a slight incline of 1 in 24 in a level. At a point about 30 yards further inbye two loaded tubs were standing unattached against a loaded tub which was clipped on behind them. When the rope started the two loaded tubs were pushed forward and gravitated down the incline. Deceased got into the way of them and was bodily crushed. Pneumonia supervened and he died 15 days later. Before starting, the rope and tubs should have been examined to see that all was safe.
