He was employed in emptying shale at a tip. After partly emptying a wagon by the side door he probably tried to release one of the bottom doors, but was only partly successful. He was last seen shovelling from the inside of the wagon, and an hour later was found beneath the wagon covered by the material that had come through the bottom door which had dropped down.
