Deceased was filling into a waggon some overburden which had fallen from a face 12 feet in height which had previously been observed to be bulging. A further fall took place which caught his legs and knocked his head against the wagon, causing injuries which resulted fatally three hours afterwards. The fall had come away from a sand pocket and was probably due to the presence of a drain pipe. Deceased was in charge of the uncallowing gang, but neither he nor anyone else appeared to have considered it necessary to examine the top.
