Details of Event
Deceased was digging out the foot of a gate pack to rebuild it. A fall of side occurred, followed by a fall of roof burying him and breaking his neck. The gate was set out parallel to the face, consequently the weight breaks ran along the gate. No bars or stretchers were used. Temporary props should have been set to hold the roof until such time as the new pack had been completed.
