He pushed a loaded tub forward at mid-landing, evidently thinking the cage was at the landing, opened the gate with one hand and held it open while he pulled the tub towards him, and as it passed him he got behind it to push it. He and the tub fell to the bottom of the shaft, a distance of 210 feet and he was killed instantly.
