Inspection made. Whilst taking a tub of dirt downbrow one person had hold of the tub on the upper side and there were two scotches in the wheels and a sprag at the front. Deceased it appears took the sprag out and kept in front of the tub some distance until it ran against the side. He was asked to come from the front of tub for the other man to put the tub right but he appeared to have done so and when the full tub was clear it got over them and ran over his leg and was fatally injured. Breach of Special Rules.
