- A fall of roof had occurred on the main haulage crut near the shaft where it intersected the Bullhurst seam at a place where old dips had been driven in the Bullhurst. This fall caused a cavity in bad ground over the roof of the crut the roof and sides of which cavity were not properly secured before the crut was retimbered. After the crut had been retimbered by five large bars with runners and laggings over the bars a large fall from the roof and sides of the cavity broke down or reeled out the timber and filled the crut for 7yds while a number of men were at work finishing the repairs. Loyden, Shaw, and Farr were dead when got out and Turner died on the 20th September. An under-manager and overman were also injured by the fall. There was a grave error of judgement on the part of the officials in charge of the work in not first properly securing the roof and sides of the cavity above the crut.
