- Deceased had gone down a jig 90 yards in length, and having an inclination of 1 in 2.5, for the purpose of lifting on a derailed tub. Having done so, he got into a manhole and rapped the tub away. He remained in the manhole until several tubs had been run, and then, during a pause, went up to the manhole nearest to jig top. Another loaded tub was then run down, and when the empty tub was landed he left the manhole to ascend to jig head. As he did so, the empty tub came back, struck, and killed him. The jig head was secured by a block 7ft. 9ins. in length and 4.5ins square, laid on the floor across the whole width of the landing, one end resting against a post and the other against a chock. It had apparently not been properly placed, and on the empty tub being pushed back against it, the former end slipped off the post and the tub ran amain.
