- He was engaged fixing some guides in a shaft and stood on a platform to do so. As some work was about to be done at the top of the same shaft he was told to fix a covering up above where he was working. This he did not do but two other men had almost finished doing so when an alarm was raised that something was falling down the shaft. He became flurried mid instead of stepping into the ladderway vaulted off the platform at the open side and fell down the shaft. Had he remained still he would not have been hurt, as the thing haft was a thin piece of wood of no weight. If he had been carrying out the orders given to him he would not have been hurt as he could have gone back in the adit level from the shaft. It would have been very much safer if all the work in the shaft had been completed before anything was done round about the mouth on this occasion.
