Deceased, with others, was at the shaft bottom waiting to ride. Before the descending cage came to rest in the bottom a chain across the mouthing was knocked out of its staple, and the men crushed towards the cage. The man actually got into the bottom deck, and deceased got partially in, or was pushed in he then appeared to have been trying to get out again, but was caught between the floor of the top deck of the cage and the staging and fatally crushed.
