Deceased was barring off a rate of coal from the long-wall face, when the rate came off suddenly, followed immediately by a heavy fall of the roof-coal and dirt above. He was knocked down and so severely injured that he died on the same day. After the accident a slip was exposed at the back of the rate of coal, and this extended up into the roof coal the fall came from between this slip and a break along the edge of the face tree lids.
