He had holed a shale face for a length of 8 feet and to a depth of 1 foot. After trying to get the holed shale down he went to fill it tub opposite the face which was not holed. Whilst he was doing this the holed shale fell, but the fall extended beyond the holing and he was buried. He was so seriously injured that died the following day. The fall disclosed a previously unsuspected vertical joint or slip.
