- Deceased was killed by a fall of roof at a longwall face in the Brooch Coal Scam. He was knocking out a sprag at the centre of a length of about 4 yards of face which had been holed to a depth of 2ft. The coal fell over from a slip and knocked out two props standing close to the coal and allowed the roof to fall for a distance of 5 yards. The roof fell from a hanging slip previously invisible, parallel to the face, and from a break running along the ends of the last row of bars set to keep the roof up over the wagon way along the face. The place was well timbered while the coal was up and the sprags were supporting it. The mistake was in attacking so great a length of coal as 4 yards, from the middle instead of from either end.
