The accident occurred at the pump inset in No.2 Pit, 130 yards from the surface. Deceased and the manager had come from the shaft bottom to the pump inset by means of the cage about 20 minutes before the accident took place. Meanwhile the cage was liberated and used for winding coal from the bottom. Having inspected the pump they signalled for the cage which was then lowered to the pump house inset. The usual signals when men are about to ride having been given and returned from the surface, the manager entered the cage and stood at the far end while the deceased man gave the final signal before stepping into the cage. Deceased duly signalled and was in the act of stepping into the cage when it was unexpectedly raised and upset him so that he fell to the bottom of the shaft and was instantly killed. The manager tried to save him but failed to reach him. Insufficient time was given by the engineman after the signal was given to enable deceased to enter the cage before it was raised from the inset.
