- Deceased was crushed between the buffers of an empty and a loaded railway wagon. In order to start the loaded wagon moving from under the screen he got between the buffers and pushed on the empty with his back against the buffer of the loaded wagon. Whilst he was doing so a shunter allowed another empty to come down the siding and to bunt into the top end of four standing next the one deceased was pushing on. Before bringing the empty down the siding which was inclined at 1 in 60, the shunter had seen that no one was about the wagons. There is a slight curve at the spot and deceased and the shunter could not see each other until the empty almost reached its destination. Since the occurrence an electric alarm bell has been put up and it is switched on to ring and to continue ringing all the time that wagons are moving on the siding.
